  • Once you are scheduled for your first appointment, you will receive a welcome email with a link to initial forms that can be filled out online. You will be able to use this online portal ongoing for sharing any confidential documents and communications. Please also bring a copy of your insurance card.

  • Anchored Therapy is located in the heart of Lincoln Square. It is a 3 minute walk from the Western Brown Line Stop. There is metered street parking on Lawrence Ave. directly in front of the building or on Lincoln Ave.

  • The office has an elevator.

  • Sessions can be cancelled or rescheduled when needed. 24 hours notice is requested for any changes to the session time. Late cancellations or no-shows will be charged the full fee of the session.

  • I am in-network with BCBS IL PPO and Aetna. It is recommended that you call your insurance company prior to the first session to verify benefits.

  • If you have out-of-network coverage with a different insurance, you will receive a “super bill” which is an invoice that you can then submit to your insurance for reimbursement for sessions.

  • Intake: $180

    Individual Session 90837: $165

    Individual Session 90834: $145

    Individual Session 90832: $100

  • Sliding-scale options are offered on a limited basis. Please indicate when scheduling if you are interested in discussing a reduced fee.

  • Checks, Credit Cards (except Amex), Venmo, Zelle, and cash are all accepted.
